Leukemia Patient Financial Aid Assistance

If it is difficult or impossible for you to afford drug co-pays or your health insurance monthly payments, you may be eligible for assistance from The Leukemia & Lymphoma
Society (LLS).
Who is Eligible for The Leukemia & Lymphoma Society (LLS) Financial Aid Assistance Program?
You are eligible if you meet the following criteria
Has a household income that is at or below five times the U.S. Federal Poverty Level.
Has prescription drug insurance coverage such as:
Private Health Insurance
Medicare Plan B or D
Medicare Supplementary Health Insurance
Medicare Advantage
Eligibility for this program is also based on fund availability for specific blood cancer diagnoses and is subject to change.  To get a current list of available finds by blood cancer diagnoses visit www.LLS.org/copay or call (877) 557-2672
You will need to fill out a complete application with your doctor's signature and information on your diagnosis for blood cancer.  You will also need copies of you last income return, W2 form or other proofs of income.  If you didn't fill out an income tax return, they will accept your W2 forms.
